FinalForms @ SVT
SVT uses FinalForms for field trip permission slips (for all) and open campus lunch (for juniors and seniors only). To access FinalForms:
2. Log in or create an account if you don't already have one. If your student has previously participated in sports or clubs with CVSD, you should already have an account. If you have an account with another school district, you will still need to create a secondary account with CVSD using the portal link above. You may use the same email address for both accounts.
3. Select the clubs and/or skill center program your student will participate in this year. You can update these selections throughout the year as needed.
4. Complete all required online forms. Once filled out, your information is saved from year to year, requiring only a brief annual update each fall.
5. If your student lives within CVSD boundaries, they may automatically connect to their boundary high school, meaning you might not initially see SVT-specific forms. If this occurs:
- Register your student on FinalForms as outlined above.
- Email your student’s name and the school they are connected to our SVT Head Secretary, Shawna Mallonee, at [email protected]. She will update your access to include SVT (your student will retain access to their home high school as well).
- Once SVT access is granted, log back into FinalForms to sign the SVT-specific Forms.
